Topics Prescription Search:  
Introduction: The Prescription screen enables users to prescribe drugs to patients and send prescriptions to pharmacies directly. Drugs can be prescribed from this screen only for patients with open encounters. Drugs prescribed in one encounter can be automatically copied to the next encounter, saving providers the effort of re-entering drugs for the current prescription. This can be done by turning On the property rx.autocopy.lastenc.drugs from the Properties Master screen. All prescription details from the last encounter – except the Diagnosis code – are automatically copied to the prescription of the current encounter.

Note:
  • If the previous encounter is a zero duration encounter (including RR – Refill Request and SR – Surescripts Refill), the prescription from that encounter is ignored. Instead, the prescription from the encounter prior to the zero duration encounter is copied to the prescription of the current encounter.
  • The prescription from the previous non-zero encounter is NOT copied when a new consecutive encounter starts. It is copied only when the Prescription screen is accessed within the current encounter. This includes accessing the Prescription screen from the Doctor Forms screen, the Assessment screen, CPOE → Rx, and Encounter TOC → Prescription.
  • The prescription from a deleted encounter cannot be copied, and the prescription from a reopened encounter cannot be considered.
  • If no prescription was entered in the immediately preceding encounter, the system automatically retrieves the prescription from earlier encounters.

Add Drugs icon: Clicking this icon opens the Drug Search popup, which displays a list of preferred dispensable drugs with their names and strengths.
  • There are four tabs on the Drug Search popup as follows:
    1. Dispensable Drug Pref - Preferred
    2. Dispensable Drug - All
    3. Order Set
    4. Comprehensive Drug Search
    Note: This feature must be enabled conditionally using a property. Note that the applicability of InHouse support, as determined by the property, is based on the logged-in location for MultiLocation versions.

    After selecting a drug from the Drug Search popup and clicking the Ok button, the drug is added to the Prescribed Drugs Table.

    Add Inhouse Drugs icon: Click this icon to see a list of Dispensable Inhouse Drug Names. If the selected drug is available as In House, the In House checkbox is automatically selected.

    Pediatric Dosage Button: This button appears when the patient’s age is under 18. It displays the calculated pediatric dosage per day. The calculation is based on the patient’s age and weight. The system uses the weight entered on the Vitals screen in the current encounter. If no weight is entered, the system uses the Normal Average Weight for the patient’s age and sex. The minimum and maximum dosage per day is calculated and displayed. Providers can use their discretion to decide the dosage and frequency accordingly. This is an informational display only, which users can access as needed. It does not prevent or warn users from prescribing any drug.

    Past Rx: Click on this icon to open the Current Medication view as seen on the Facesheet with all its functionality in a popup, listing the ‘Old Med’ drugs, ‘Stopped’ drugs, and ‘Over’ drugs.

    Medication History: Displays the medical history details. Enables a global search across the Pharmacy and PBM tabs in Medication History (and Current Medication), highlighting both tabs in yellow when a match is found in both, and in red when found in only one

    Past Medical History: Click on this icon to open a PMH popup displaying the patient’s present medical history information added in the encounter.

    Prescription Action Panel

    Lab Matrix icon: Click on the icon to view Lab Matrix details in a popup.

    Vital Matrix icon: Click on the icon to view Vital Matrix details in a popup.

    Order Sheet icon: Click on the icon to view Order Sheet details in a popup. This icon is automatically hidden when the user has already opened the Prescription screen from within the Order Sheet.

    Compose icon: Click on the icon opens a popup that allows you to compose a message.

    Patient Details icon: Click on the icon to see Patient's registration information in a popup format.

    Patient Alert icon: On page load, the Patient Alert popup does not open automatically. Users must click the icon to open the popup. When clicked, the Patient Alert popup allows users to view existing alerts for the patient and create new alerts. The Patient Alerts option is used by front office staff to manage alerts for patients who miss appointments or fail to make timely payments. Staff can enter comments in the popup and define a valid date range for the alerts. Additionally, Patient Alerts can be deleted directly from the Patient Alert popup. After alerts are saved in this popup, the icon changes to .

    Patient Appointment History icon: This icon is present only on the Rx and Refill screens to allow users to access patient appointment history details. If the patient has no previous appointments, this icon is displayed as . Once any appointments are scheduled for the patient, this icon is displayed with a red indicator as . Clicking this icon opens the Appointment History popup, which displays a list of all scheduled appointments for the selected patient. Appointment details are displayed in the following columns on this popup:
    • Date: Displays the date on which the appointment was scheduled.
    • From: Displays the time at which the appointment was scheduled.
    • Upto: Displays the time up to which the appointment was scheduled.
    • Status: Displays the appointment’s status.
    • Provider: Displays the name of the Attending Provider for the appointment.
    • Encounter Type: Displays the encounter type selected for the appointment.
    • Comments: Displays comments added for the appointment, if any.
    If the patient has no previous appointments, a blank Appointment History popup is displayed. A Manage Columns icon is available in the top right corner of the popup to reorder the displayed columns. Appointment history details can be printed by clicking the Print icon. The Appointment History popup can be closed by clicking the OK button.

    Eligibility Button: Users can select one from multiple eligibilities returned by Surescripts for the patient by clicking this button.

    Preview icon: Click to preview prescription details before faxing, printing, or sending. Prescriptions containing only In House or Pharmacy drugs can be previewed. A list of all applicable templates is available in the Template dropdown. For In House drugs, all templates with names starting with In House are considered. For other pharmacies, all templates (including those for In House drugs) are considered. Users can select a template and then print or fax the prescription.

    Print icon: Click to print both In House and other Pharmacy prescriptions. Both appear in different formats and templates. (For In House dispensing, the Pharmacy Name and Address must not be printed.) The In House Prescription uses the first prescription template alphabetically whose name starts with In House. The Non-In House prescription uses the first prescription template alphabetically, with no constraints on its name. Depending on whether the property allow.directprint.druginfo is turned ON or OFF, clicking the print button either sends the Drug Info PDF directly to the default printer or displays a popup for printer selection.

    Fax icon: The prescription can be faxed to the Pharmacy by clicking this icon. In House drugs cannot be faxed. Users must select the drugs and save the information before faxing.

    eRx icon: Drugs eligible for eRx can be sent electronically by clicking this button. In ePrescription mode, the prescription is sent electronically directly to the selected Pharmacy. Clicking the eRx button opens the eRx Summary popup, which contains Patient details, Prescriber details, Drug details, and Pharmacy details. If Prior Authorization is required for a drug in the prescription, clicking the eRx button displays an Electronic Prior Authorization warning message with the buttons ePA, eRx, and close. The user can proceed to send the eRx by clicking the eRx button on this popup.

    Note:
    • Drugs eligible for eRx cannot be printed or faxed until they have been sent electronically.
    • Only ICD10 codes associated with the drugs are displayed on the eRx Summary popup as well as in the XML sent to the Pharmacy when drugs are sent via eRx. ICD9 codes are not displayed. The ICD10 codes are displayed without a decimal point in the ePrescription (for example, ICD10 code R50.83 is displayed in the ePrescription as ICD10:R5083).
    If the admin-level property rx.rxinform.enable is turned On from the Properties Master screen and the Allow text messages on cellphone checkbox is selected for the patient on the Patient Registration screen, a Provider consent checkbox is displayed before the action buttons on this popup. This checkbox is selected by default and reads: Monitor Rx adherence and text supporting information to patient at: followed by an editable text field where the patient’s cell phone number is auto-populated. This checkbox is a mandatory requirement for sending RxInform text messages to patients. These text messages contain essential information regarding prescriptions and refills, including drug pricing, discounts, and educational content related to their medications.

    If the Provider clicks the send button without deselecting the consent checkbox, the prescription is sent and an RxInform text message is delivered to the patient’s cell phone number. If the Provider deselects the consent checkbox before clicking the send button, the prescription is sent but no text message is delivered to the patient.
    Note:
    • RxInform text messages are not sent to minor patients under the age of 18.
    • Currently, if the patient’s cell phone number is not SMS-enabled and the provider selects the checkbox and sends the prescription, the patient does not receive any text message.
    Cancel eRx icon: Users can click this icon to cancel an electronically sent prescription. Clicking the Cancel eRx icon opens the Cancel/Discontinue eRx popup, which includes Patient details, Prescriber details, Pharmacy details, and Drug details for cancellation. Depending on the request type selected, the Provider may cancel the entire prescription or discontinue the refills from this popup.

    If Print Parameter 18 is used in the Prescription Parameter properties rx.rx.fields, rx.brf.fields, and rx.det.fields, canceled drug details – suffixed with (Canceled eRx) in red and bold font – are displayed in the Progress Notes for the encounter. Print Parameter 18 cannot be used in the Prescription Parameter property rx.ccd.fields, as CCD does not contain canceled drug information.

    ePA: Click the ePA button to open the Electronic Prior Authorization popup. This popup has two sections: Drug List and Surescripts Accelerator.

    The Drug List section displays those drugs in the prescription for which prior authorization is required. The following are the columns present in the Drug List section:
    • Select: Users can check the checkbox associated with a drug to select it.
    • Drug Description: Displays the details of the drug in the prescription for which Prior Authorization is required. The details include the Drug Name, Strength, and Frequency. Users can also add drugs to the prescription directly by clicking the + icon.
    • Request Dt: Displays the date when the Electronic Prior Authorization request was submitted.
    • ICD codes: Displays the ICD codes associated with the selected drug. ICD codes from the Assessment screen can be associated with drugs in this popup directly using the multi-select button. Only ICD10 codes are displayed in this column. ICD9 codes are not displayed.
    • To Rx: If ICD codes are assigned to a drug from this popup and the To Rx checkbox is selected for that drug, those ICD codes are associated with the drug and are reflected on the Prescription screen.
    • Priority: Users can check this checkbox and then click the ePA REQUEST button to notify Surescripts that the ePA request is urgent.
    • PBM: Displays the Pharmacy Benefit Manager name from the selected Eligibility.
    • Plan:
    • Displays the Plan name from the selected Eligibility.
    • Response: Displays the response to the ePA request as received from Surescripts. These responses may be: O:Open, A:Approved, D:Denied, P:Partially Denied, I:In Process, and C:Close
    • Reason: Displays the reason for rejection as received from Surescripts.
    • Note: Displays notes or comments received from Surescripts.
    • PA No.: Displays the Prior Authorization Number received from Surescripts once the ePA is approved.
    • ePA REQUEST button: Click the ePA REQUEST button to send an ePA request for a drug to Surescripts.
    Note: Prior Authorization Workflow for a drug applies only to patients with a valid Eligibility on file.

    Properties: This button is visible only to users with Read Access to the Properties Master screen. Access to this button is granted to enhance user functionality. Clicking the Properties button opens a popup that lists all prescription-related properties from the Properties Master screen in PrognoCIS.

    Prescription Table

    Inhouse checkbox: Select this checkbox to choose an drug. The drug list will then display only InHouse generic drugs. If the selected drug is available as In House, the In House checkbox is automatically selected.

    Sample checkbox: If this checkbox is selected, the drug is assumed to be dispensed InHouse, even if it is not part of the defined InHouse drug list.

    Drug: Users can type the name of the drug to be added to the prescription in this field. As characters are entered, drug names are displayed in the dropdown list. The list of drugs displayed depends on whether the InHouse checkbox is selected and whether the property rx.show.inactive.drugs is turned On or Off. Compound drugs are also displayed here. Clicking a drug name from the list automatically populates the default values for Strength, Dosage, Frequency, Days, Bulk Quantity, and Refills.

    Substitute checkbox: When checked, instructs the pharmacy to dispense a substitute drug if the prescribed drug is unavailable.

    Strength: This field displays the strength information of the selected drug. The strength list may display an asterisk (*) suffix next to an entry to indicate it is inactive.

    Drug Info Button: After entering both the drug and its strength, click this button to view detailed information available in FDB, including: How to use, Side effects, Drug interactions, and Min/Max Daily Adult Dose. A print button is also available at the bottom; clicking it generates a PDF preview of the Drug Information for printing.

    DEA Class: If the selected drug falls under a DEA class, its class is displayed here in red.

    Dosage Unit: Also known as Dispense Unit, this is a dropdown list of standard units supported by Surescripts. Only one dosage unit can be selected at a time, and users can define additional units if required. However, note that only Surescripts units can be selected for Bulk Quantity. If a non-Surescripts dosage unit is selected, it is not set as the Bulk Unit and the Bulk Qty is not calculated.
    Note: For any oral liquid drug, it is mandatory to select Milliliter as the Dosage Unit.

    Dosage: Specifies the amount of the drug to be taken at one time. Any number, fraction, or text such as Half can be entered in this field. For example, 2 OR 0.5 OR Half.

    Frequency: It is displayed as a dropdown list of codes or names based on the property rx.freq.byname. If the selected drug has not been used before, the default frequency will be set to daily. The selected frequency is used to calculate the bulk quantity. In the case of ointments, a frequency may not apply. In such cases, users can select *NA* (for Not Applicable), and the bulk quantity will not be calculated.

    PRN checkbox: This checkbox indicates that the patient may take this drug on an as-needed basis. The label for this field can be changed using the property rx.prn.label. This checkbox is unchecked by default.

    Long Term checkbox: Providers can check this checkbox to keep drugs visible as part of the current medication list even after the prescribed period ends.

    Days: This is the number of days for which the drug is prescribed. This must be entered as a whole number.

    Bulk: Also referred to as Dispense Quantity, this is calculated for tablets, capsules, and caplets ONLY. To calculate this, the system uses the integer portion of the entered dosage. If the entered dosage is a valid number such as 3 or 0.5, the calculation will be accurate. For dosage values such as ½, it computes based on 1; for the text "Half", it computes as zero. Users can edit the bulk quantity if needed. Quantity is always set to 1 for other types of Dispense Units (Liquids/Ointment).

    Dispense Unit: By default, this is the same as the dosage unit. If a different unit is selected, the system will not calculate the quantity. Note that unlike the dosage unit field, user-defined units are not considered here – only Surescripts-defined units are accepted.
    Note: For any oral liquid drug, it is mandatory to select Milliliter as the Dispense Unit.

    Direction: Direction is a combination of dosage, dosage unit, frequency code or name, and number of days. This is a user-editable field with a character limit of 255 characters. Users can click the icon to open a larger popup for easier editing. The default format is Dosage+Dosage Unit+Frequency+Days. Frequency can have a value of C (for Code) or N (for Name); it will be displayed as TID or Thrice a day accordingly. Days can be:
    • Blank to display Blank (i.e. for X Days not included in direction)
    • FD for X Days
    • DD dispense X Days
    When adding a new drug on the Prescription screen, the Direction field must be filled with valid data. If the field is left blank, a message displays: Please enter direction. When the user fills in the Direction field, the following rules apply:
    • Single characters are not allowed.
    • The following sequences are not permitted: "-", "--", and "---".
    • Numbers-only input is not allowed.
    If any invalid entry is detected, a popup alert appears with the message: Invalid direction. Clicking the save button in this case displays a message listing all drugs with invalid directions along with their row numbers. This ensures that no drugs with invalid directions are saved.

    Pharmacy: By default, this is the pharmacy from the patient’s most recent prescription. This field is optional and can be left blank, as a patient may prefer to use any pharmacy while traveling. When a pharmacy name is typed, a list of matching names from the master records is displayed for selection. If the selected pharmacy supports electronic prescription receipt, a prefix of eRx is displayed; if it supports electronic prescription of Controlled Substances, a prefix of EPCS is displayed; and if it supports eRx cancellation service, a prefix of Cancel-eRx is displayed.
    Users can use the Search Pharmacy button to select a pharmacy from the master list of pharmacies provided by Surescripts. Not all pharmacies listed here are available through the auto-complete control described above. Search results are based on geographic location (City, State, and Zip) and include Mail Order pharmacies, among others.

    Pref Pharm button: This button opens the Patient Pharmacy popup, which displays a list of all Preferred Pharmacies selected for the patient, along with two icons:
    • Patient Preferred Pharmacy: Displays a list of the patient’s preferred pharmacies. If a pharmacy is selected from this list, it is displayed in the Pharmacy field.
    • Clinic Preferred Pharmacy: Displays a list of Clinic preferred pharmacies. The list is added from menu: Settings → Configuration → Vendor column →Pharmacy.
    A patient’s preferred pharmacies can be added in two ways:
    • Patient Registration Screen: Click the Preferred Pharmacy icon and select up to 10 pharmacies from the corresponding selection popup.
    • Prescription Screen: Click the add new button in the Patient Pharmacy popup to open the Pharmacy Vendors search popup. Select one or multiple pharmacies from the list, then click OK to add them to the top of the Patient Pharmacy popup list. These pharmacies are automatically added to the Patient Preferred Pharmacy list, accessible from the Patient Registration screen.
    Note:
    • Pharmacies added from the Prescription screen cannot be deleted directly from the Patient Pharmacy search popup. To remove any pharmacies, users must delete them from the patient’s record on the Patient Registration screen.
    • If a non-Patient Preferred Pharmacy is selected via the List of Pharmacies icon and associated with a prescription, that Pharmacy Vendor is added to the Clinic’s preferred list when the prescription is printed or sent via eRx.
    Address icon: Clicking this button opens a Vendor Address popup that displays the address of the selected pharmacy. The address can be edited if needed.

    Pharmacy Service: The Service column on Pharmacy Search displays New Rx as New and EPCS as Controlled Substance .

    Pharmacy Notes: Pharmacy notes of up to 255 characters can be entered in this field. Users can click the icon to view and edit Pharmacy Notes in a larger popup.

    Diagnosis : Click the Multi Select icon next to this field to open the ICD Mapping (Assign ICDs) popup and associate primary and secondary ICD codes with the drug. The first selected ICD code is treated as Primary and the second as Secondary by default. This can facilitate later analysis reporting on drugs prescribed for various diagnoses. The tooltip displays the Primary and Secondary ICD10 Codes, Description, and SNOMED Codes. An Add ICDs to Assessment button is available on this popup; clicking it opens the Diagnosis Codes popup. Users can select up to 10 codes and add those ICDs to the prescription. The ICDs selected from this button are Assessment ICDs only and are shown on the Assessment screen. An Add Past ICD Codes button is also available on this popup; clicking it opens the Add Past Medical History ICD Codes popup. Users can add ICD Codes from the Past Medical History screen to the prescription.

    If the rx.assign.icd.popup property is set to On, and one or more prescribed drugs do not have an associated ICD code, clicking the save button opens the ICD Mapping (Assign ICDs) popup. This popup lists all prescribed drugs without an assigned ICD code. Users can add ICD codes by clicking the Add buttons, selecting the checkboxes for the appropriate ICD codes, and clicking OK to associate them with the corresponding drugs. The popup also includes an Assign top 2 ICDs to all drugs checkbox, which automatically assigns the top two ICD codes to all listed drugs.

    Reason: This field allows users to enter a justification of up to 50 characters. Click the Edit Reason Search to invoke the Reason Search popup

    Fill Date: This field allows users to specify a date on or after which the pharmacy should dispense the drug. If no date is specified, this date defaults to the Encounter Date when the save button is clicked on the Prescription screen. If the drug’s Valid Upto date is greater than or equal to the Encounter Date, the RxDate after refill will be the Valid Upto date; otherwise, the RxDate will be the Encounter Date.
    Note:
    • The related Valid Upto date is calculated for a refilled drug only when the No. of Days is present.
    • When providers need to prescribe a prescription intended to start on a future date, the system sends this as additional information in the comments to Surescripts during ePrescribing. The user is first prompted with an alert indicating that the Prescription Start Date is a future date, and asked whether they would like to include this information in the comments during ePrescribing. If the user confirms, the Start Date will appear under the Comments column in the ‘Rx(s) for transmission’ popup. Clicking ‘Send’ ePrescribes the selected drug and sends it to the relevant pharmacy.
    Drugs cannot be sent via eRx if the Fill Date is earlier than the current system date. If the Fill Date for at least one selected drug row marked for eRx is earlier than the current date, a warning message is displayed when the save button is clicked. This message asks the user to ensure that the Fill Date is on or after the current date. It also includes the row numbers of the affected drugs for easy identification and modification.

    If the eRx button is clicked for drugs with past Fill Dates, an error message is immediately displayed identifying the affected drugs. This message is displayed in a popup with Ok and Cancel buttons. If the Ok button is clicked, the popup closes and the Fill Date is updated to the current date for each selected drug row and saved. The user must then click the eRx button again to send the prescription via eRx. If the Cancel button is clicked, the popup closes and the user can update the Fill Date to the current date or later and then click the eRx button again to send the prescription via eRx.

    Valid Upto Date: The pharmacy should dispense the drug only up to this date. This is not the date up to which the medication is to be taken by the patient.

    Refills: This is the number of permitted refills for the drug that the patient can obtain from the Pharmacy.

    Prohibit Refill Request checkbox: This checkbox determines whether a Refill Request can be sent for the selected drug. If this checkbox is selected for a prescribed drug, Refill Requests for it are not expected from the Pharmacy. This checkbox is independent of the Refills text field.

    No of Units: These are the Billable Units relevant for InHouse drugs. The associated CPT/HCPC Codes are transferred to the Assessment screen with these units.
    Note: If the product of the No of Units field exceeds 5 digits, the No of Units field value is reset to 1. Additionally, if the No of Units product exceeded 5 digits for existing records, the CPT code is not added to the Assessment screen for those records.

    Deny Checkbox: Users can select this option to notify the Pharmacy that the drug has been denied and should not be dispensed to the patient.
    Note:
    • If this option is selected for a drug, an Update adds the prefix ‘Denied’ to the Pharmacy Notes entered.
    • A denied drug can be printed or faxed, but cannot be sent via eRx. Denied drug names are printed with the prefix ‘Denied’.
    • Denied drugs are not displayed on the Current Medication list, but are part of the patient’s Past Rx.

    Formulary Status / Copay / Coverage Info: At the start of the encounter, the system retrieves Eligibility Information from Surescripts for the selected patient. (Note: It is not necessary to have entered any patient insurance.) There can be multiple Pharmacy Benefit Eligibilities received from Surescripts. Using the first eligibility from the list, the program displays the Formulary Status, Copay, and Coverage details for the selected drug. If the user has selected an alternate eligibility by clicking the Eligibility button, that eligibility will be used to determine the same details for the selected drug.

    Formulary Info icon: Click on the Formulary Info icon to send an RTF/RTPB request. The formulary information received in response is then displayed in the Real Time Formulary and Prescription Benefit Information popup.

    Rx Coupon icon: This icon is displayed on the Prescription screen only if the property rx.coupon.enable is set to On. If this property is set to Off, the Rx Coupon feature is disabled and this icon is not displayed on the Prescription screen.

    Clicking this icon opens the Rx Coupon/Enrollment Details popup to view applicable coupon details for the selected drug. If no drugs have been added, clicking this icon displays the message: No information to show. If a drug has been added and the save button is clicked, a $ indicator appears to the right of the drug name if any coupons or offers apply. If the prescribed drugs are from an encounter whose visit date is more than 3 days old, the system does not check for available offers for the drug. The color of the $ icon next to the drug name determines the type of Rx Coupon that is applicable:
    • If a Cash Offer is applicable, a green $ indicator is shown.
    • If a Copay Offer is applicable, a blue $ indicator is shown.
    • If a Patient Enrollment Program is applicable, an orange $ indicator is shown.
    • If Patient Support is applicable, a yellow $ indicator is shown.
    The Rx Coupon/Enrollment Details popup has two buttons: CLOSE and PRINT. Clicking the CLOSE button closes the Rx Coupon/Enrollment Details popup. Users can print the Rx Coupon details by clicking the PRINT button on this popup.

    Note: When the property rx.coupon.enable is set to ON, Rx Coupon details can be sent to the patient via email or text based on the property rx.coupon.medium. Based on the selection in the property rx.coupon.medium, the system shares either the cell phone number, email address, or both of the patient with OPRX for sending the coupon details to the patient. Delivery of Rx Coupon details to the patient on the selected medium depends on the Text/Email feature available at OPRX.

    Prescription Columns

    The bottom half of the Prescription screen displays all the drugs prescribed to the patient in the encounter. The current row is displayed with a light blue background. Clicking on a row transfers all drug details to the top part of the screen for editing. The display of last prescribed drug details depends on the property rx.default.dispensable.fields.blank. If this property is turned On, users must manually enter drug details each time.
    Note: The property rx.default.dispensable.fields.blank overrides the currently existing property rx.std.patient.chartno. in the system.

    Drug details are saved and moved to the Prescribed Drugs Table when the following actions are taken:
    1. The user adds a drug and clicks the update button.
    2. The user adds a drug and presses the Enter key on the keyboard.
    3. The user adds a drug and directly clicks the save button without clicking the update button. After the drug is saved, the user is automatically navigated to the first row in the table and the save button is disabled until the user adds or modifies another drug.
    4. The user adds a drug and clicks on another row in the Prescribed Drugs Table without clicking save or update. After the drug is saved, the user is automatically navigated to the row they clicked on. The save button remains enabled in this case and the user can add or modify another drug.
    5. The user adds a drug and clicks the Past Rx button. The drug is saved, and the Medication List popup opens.
    Select Checkbox:
    • Mark the drugs in the prescription that need to be sent to Printer, Fax, or eRx.
    • By default, a newly added drug is selected. Note that if multiple selected drugs are sent, they will all be sent to the Pharmacy.
    Type:
    • This column displays the type of the drug – whether it is InHouse, eRx, Signed, or Not Signed. The following types apply to change requests, cancel Rx, and Rx Fill:
      1. CHGREQ - Change request received for a drug from Pharmacy to Prescriber.
      2. CHGRES_A - Approved change response for a drug sent by Prescriber to Pharmacy.
      3. CHGRES_C - Approved change response for a drug with changes sent by Prescriber to Pharmacy.
      4. CHGRES_D - Denied change request sent by Pharmacy to Prescriber.
      5. CANREQ - Cancel request sent for a drug from Prescriber to Pharmacy.
      6. CANRES_A - Approved cancel response for a drug from Pharmacy to Prescriber.
      7. CANRES_D - Denied cancel response for a drug from Pharmacy to Prescriber.
      8. RXFILL_F - Rx fully dispensed from Pharmacy to Prescriber.
      9. RXFILL_PF - Rx partially dispensed from Pharmacy to Prescriber.
      10. RXFILL_NF - Rx not dispensed from Pharmacy to Prescriber.
    Drug Name:
    • Displays the Drug name and Strength.
    • Drug names are displayed in Bold if they are InHouse. A check mark prefix is shown if the drug has already been sent.
    • A red prefix is displayed if eRx is applicable.
    • Formulary status of the drug is displayed with a Formulary Status: label in black and the status in blue. The formulary status is also displayed in the tooltip.
    • Prescribing a Controlled Substance (CS) drug involves additional steps for ePrescribing. The Prescriber must sign the Prescription and the Prescribing provider has to keep the eToken connected to the USB drive of their PC (Personal Computer).
    • DEA Class II drugs can be dispensed only once from the Prescription screen.

    Direction: The default or edited direction for the drug.

    Pharmacy Notes: Drug comments entered, if any.

    Disp: Dispensable Quantity

    Refills: Refill count

    Last Fill Date: Refill count

    Pharmacy: Name of the Pharmacy is shown here.

    Actions: Shows the Delete icon. This icon turns red when clicked. Users can select multiple rows and click Save to delete multiple prescriptions at once. For pharmacies with Cancel eRx service, the Delete icon is disabled for drugs sent via eRx even if the property rx.edit.sent.drugs is set to ON.

    Action Buttons:

    Save: Users can click this button to save a drug added or modified in the patient’s prescription. If any drug detail fields are blank – that is, if some drug details are missing – appropriate alert messages are displayed when this button is clicked. Similarly, if any validations are applicable for the drug, appropriate validation messages are displayed when the save button is clicked. The drug is moved to the Prescribed Drugs Table only after the missing details are entered or the validation conditions are met.

    If a clinical staff member saves the prescription, the status of the prescription changes to Entered. If a Provider or a person with Approve rights saves the prescription, the status of the prescription is set to Approved.
    • On save, the system checks for:
      1. Duplicate drug prescribed
      2. Patient Allergy to Generic Drug
      3. Patient Allergy to Dispensable Drug
      4. Allergens
      5. Cross Sensitivity
      6. Drug-Drug Interaction
    • Allergens and Cross Sensitivity defined in the Face Sheet allergy are considered. Drug-Drug interaction considers the prescribed drugs and Current Medication drugs for interaction. These checks are performed using the algorithms of FDB.
    • Duplicate drugs are not allowed. For all other allergies, only a warning is displayed and the provider is not prevented from prescribing the drug.
    • The system also checks for precautions and displays appropriate warnings.
    • If there are any unapproved drugs in the list, the user (provider) with Approve rights can approve all of them by clicking ‘Save’. The ‘Save’ button is enabled only when the Prescription screen has any unapproved drugs in the list. The user (clinical staff) without Approve rights can only enter drugs on this screen; these entered drugs are approved by the provider.
    Note: For approved drugs, their status is displayed as ‘ Approved’ ; whereas for unapproved drugs the status is ‘Entered’.

    When a Prescriber saves a prescription that contains controlled substances, a Summary screen popup is displayed showing all CS drugs that have not yet been signed. The Provider must review and mark individual drugs as ready to sign, then click sign.

    Note: The following types are displayed:
    1. Not Signed: A Controlled Substance prescription that has not been digitally signed by the provider.
    2. Signed: A Controlled Substance prescription that has been digitally signed by the Provider.
    3. eRx: A non-controlled substance prescription that can be sent electronically.
    4. Blank: The drug cannot be sent electronically. This may be because the Provider is not registered for ERX, the Pharmacy does not support ERX, or the drug is a controlled substance and the provider is not registered for EPCS.
    Reset button: Clicking the button clears all unsaved data from the Prescription screen.

    Dispense Button: This button is enabled only if an in-house drug is prescribed to the patient. A popup is displayed when the Dispense button is clicked. Basic drug dispensing information – such as the date of manufacture (Mfg), Site, Lot No, and date of expiry (Expires On) – can be recorded before any in-house drugs are dispensed. This information is useful whenever there is a need to trace back any dispensed drug.

    Order Set button: Clicking this button displays a list of all existing Order Sets. Users may decide to overwrite an existing Order Set or create a new one. Users may select any one of the Order Sets. The provider can create Order Sets of often-prescribed drugs for various common ailments.

    Access Control

    Users can reach this screen by several routes:

    a) Encounter → Prescription

    This is the most common and natural route. If the encounter is closed, the Prescription cannot be edited; it can only be viewed. If the encounter is open, the Prescription can be edited.

    b) CPOE → Rx

    If there is no current patient encounter, this option is disabled. If one is present, it is equivalent to taking the Encounter → Prescription route.

    c) CPOE → Refill The Provider or Clinical Staff would receive a call from a patient requesting a Refill. The objective is to complete the process as quickly as possible. Clicking this option displays a Patient selection search popup. Only patients who do not have an open encounter and who have at least one closed encounter are displayed. Select a patient. The system creates a new encounter for the patient with the encounter type as RR (Refill request), and the prescription screen is displayed. By default, the Rx tab will display all the drugs prescribed in the previous encounter. The provider may flag drugs for deletion if they do not intend to order them. The provider can also add supporting drugs by clicking the button, adding new rows, or selecting any drug from the past Rx tab. Note the subtle difference: the Rx tab was populated with only the drugs from the previous encounter, while the past Rx tab displays all drugs prescribed to the patient across all encounters in the past six months. This six-month period is a default property and can be changed. Save the prescription, then click Send to send it to the Pharmacy. The system automatically closes the encounter. However, if the user navigates to another option in PrognoCIS without completing the Approve and Send steps, the encounter remains open (it appears on the Provider’s Home page) and must be handled like any other open encounter.

    Note:
    • A copy of the Refill Request is also sent to the Provider’s assistant along with the recipient Provider, if the property rx.refill.msg.send2assistant is set to ‘Y’. The property overrides the existing property mail.cc.A open encounter. The Denied Refill Request (from the Portal as well as the System) is tracked and displayed individually on the Patient’s Document List (Encounter → TOC → Document List), where each denied Refill Request can be previewed individually but cannot be edited.
    • For CS drugs – For DEA Class II drugs, the user is allowed to dispense only once. The value of Total No. of dispensing approved field on Refill Request must be ‘1’.
    d) Message to Approve

    If a clinical staff member has created a prescription, the provider receives an alert to approve it. Click the message to open this screen.

    e) Message from Patient Portal Once the Patient Portal is operational, the provider will receive a Refill request message from the Patient. When the provider clicks the message, the system displays an error if there is any open encounter for that patient. If there is no open encounter, the provider will be taken to this prescription screen and a new encounter will be created. The Rx Tab will be displayed, populated with the drugs requested by the Patient. The provider can then add, remove, or edit drugs as needed, then save and send to the Pharmacy. The encounter is automatically closed, and a message is sent to the patient.

    Print Parameters

    The following fields can be defined in Prescription Parameter properties (for example: rx.rx.fields, rx.brf.fields, rx.det.fields and rx.ccd.fields) and their corresponding string or table tags (for example: [ENC_MED_BRF_STR_THIS] and [ENC_MED_BRF_TBL_THIS]) can be used to display the respective details in output templates.

    18 This displays canceled drug details suffixed with (Canceled eRx) on the Progress Notes for the encounter.
