Introduction:
When a new Patient comes into a Clinic, the Doctor or Nurse collects information on all their allergies and intolerances.
This information helps to minimize the chances of adverse reactions in a prescription.
The Allergy screen has two parts: an Allergy section at the top followed by a Transition of Care section at the bottom.
Existing and All Dropdown:
Select "Existing" from this dropdown to display the list of allergies that currently exist for the Patient.
Select "All" from this dropdown to display the entire list of allergies applicable to the Patient.
No Known Drug Allergies Checkbox:
Select this checkbox if the Patient has no known drug allergies.
Once a Drug Allergy is selected, this checkbox is unchecked and not available for clicking.
The Transition of Care section consists of allergies that have been entered via the Add Allergies button, and/or allergies imported through CCD files.
If the CCD file has been imported into an encounter of the Patient, then the details of these imported allergies are displayed from the next encounter onwards.
Add Allergies Dropdown:
This dropdown categorizes the allergies as Environmental, Food, and All.
To enter an allergy, click on the Add Allergies button to select an allergy from the Allergies search popup, which has a consolidated list of all the above allergen categories.
Check the allergy from the list to select it.
Add Allergies Icon:
Click to select the allergy from the Allergies search popup, which has the consolidated list of all the above allergen categories.
Check an allergy from the list to select it and click on OK.
If a user tries to add duplicate allergies from the Allergies popup and clicks the Save button,
then a Duplicate Allergies popup is invoked, which displays the details of the duplicate allergies and prompts the user to select one of the two duplicate allergies to be added to the Patient's Allergies list.
No Known Transition of Care Medication Allergies Checkbox:
Check this checkbox to mark the encounter as having no reconciliation done for medication allergies in the encounter.
Note: The following field descriptions are equally applicable to the Allergy and Transition of Care sections, with the exception of the Reconcile dropdown, which is present only in the Transition of Care section.
Del Checkbox:
This option, when selected for a particular row, deletes that particular row of details upon clicking the Save button.
Type:
This displays the allergy type when an allergy is selected.
Allergies are classified into six types:

Name:
If the allergy is selected from the Add Allergies button, then the Name field is populated with the selected allergy name.
On hovering the mouse over the Allergy Name, the tooltip displays the Allergy name and the RxNorm Code associated with the Allergy.
Note: For an allergy that belongs to the Allergens drug class, the tooltip displays the allergy name and the SNOMED Code associated with the allergy instead of the RxNorm Code.
Reaction:
Click on the Search Reaction button to open the Allergies Reaction search popup and select a reaction description.
You can also edit the description if required.
The Allergies Reaction search popup is enhanced with additional Allergy Reactions options, which are defined and populated from the Group Type - Allergy Reaction and SNOMED Code (AY) present under the Non-System category on the Group Types Master screen.
Allergy/Adverse Event Type Dropdown:
Select an Allergy or Adverse Event Type from this dropdown.
The following are the available values:
- Allergy to substance
- Drug allergy
- Drug intolerance
- Food allergy
- Food intolerance
- Propensity to adverse reactions
- Propensity to adverse reactions to drug
- Propensity to adverse reactions to food
- Propensity to adverse reactions to substance
Severity Dropdown:
Select the severity of the allergy from this dropdown.
The following are the available values:
- Mild
- Mild to moderate
- Moderate
- Moderate to severe
- Severe allergy
- Fatal
- Intolerance
Created On:
This displays the creation date for allergy details imported from CCD-CCR.
Status:
The possible status values are: Existing, Inactive, and Erroneous.
Source:
This displays the various sources from which the allergy has been added.
It is a mandatory field.

Last Modified:
This field displays the latest modified date, time, and time zone of the Clinic location.
Reconcile Dropdown: By default, the dropdown list displays the value Add for problems added via Ailments [+]/ICD [+] as well as for problems imported via CCD.
Note:
- The Transition of Care section consists of allergies that have been entered via the Add Allergies
button, and/or allergies imported through CCD files.
If the CCD file has been imported into an encounter of the Patient, then the details of these imported allergies are displayed in the Transition of Care section from the next encounter onwards.
- The Del checkbox is disabled for allergies imported through CCD.
- To add an allergy to the Patient's current allergy list from the Transition of Care section, the user has to select the Reconcile option as Add on the allergy.
To remove an allergy from the Transition of Care section, the user has to select the Reconciliation action as 'Remove' – this entry is not added to the Patient's allergy list.
- Once an allergy has been reconciled, that entry is removed from the Transition of Care section.
- Duplicate allergies are determined by the Type and Code of the allergy.
- The system checks for the Allergens/Cross Sensitivity/Generic Drug/Dispensable Drug conditions at the time of Prescription to ensure that the Patient is not allergic to any of the prescribed drugs.
Notes: Add notes with phrases to the Patient's allergy information. You can do this by using an Acronym. The Favorite Acronyms are shown in the autocomplete Search. Select any acronym or type the acronym and press the Tab key to populate the Allergy Notes.
You may also click the Insert Phrases search button to add a phrase from the Fast Phrases search popup.
Click the Zoom Notes icon to open and view the notes in the Allergy Notes popup.
The tag ENC_FSS_ALLERGIES__NOTES displays the Allergy Notes information on the Progress Note.
Save button: Click this button to save the details entered on the Allergy screen.
Close button: Click this button to close the Allergy screen.
History... button: Click this button to invoke the Allergy History popup, which displays the trace or log history of allergies added and/or modified by the user.
Print button: Click this button to print the Allergy details entered on the Allergy screen.
Clinic details are printed as the header, followed by the Patient details, and then the Patient's Allergy details.
