Introduction: From this screen, the user can close the current encounter. The system ensures that the encounter is complete in all respects. The table at the top shows the count of Lab/Rad Orders that are Entered, Approved, Sent, and which are Ready to E-send. Also, the count of Drugs Prescribed, Letters, Consults, and Procedures which are Entered, Approved, and Sent is displayed in the table. A setting in the property, Encounter Close Checks → enc.close.orderstatus, decides if it is essential that all orders are sent before the encounter is closed. The order is shown in red if there is a mismatch. Some doctors do not select ICD/CPT codes; other settings, Encounter Close Checks → enc.close.Icd.must and Encounter Close Checks > enc.close.Enm.must, decide if Assessment and ENM are mandatory. Likewise, a check is made on the co-payment. If the property Encounter Close Checks → enc.close.pendingordersheet is set to 'Y', then it displays the Pending Order Sheet Tests and Pending Vaccines to be Administered. The encounter cannot be closed while these are pending.
Also, an additional check condition can be applied on the Encounter Close screen to define the visibility of the Mark Encounter Complete / Sign Off checkbox from the property enc.close.assessment.must. If this property is set to a hyphen (-), then no check condition is applied here.


Note:
  • In the procedural templates of the Doctor's form, if any mandatory tests are unanswered, then on Encounter Close, PrognoCIS displays an alert.
  • The user is appropriately prompted with an alert whenever an ICD code starting with 'E' (Procedural ICD) is attempted to be marked as the primary ICD.
  • When a user closes an encounter, a letter is sent to the referring provider as a non-secured email type, which consists of a PDF file. This attached PDF file is password-protected, and its password will be sent in a trailing email.
The list boxes for Referred by Doctor and Send For Referral To Doctor have an option for Add New. If this is selected, the user can create a new referring doctor in a popup. The doctor can ask the patient to come back for a follow-up after the specified number of days selected from a list box and select the follow-up reason from the search icon. The Attending Doctor can refer the patient to any other doctor. The attending Clinical Staff selection can also be changed.

Review: Any user can click on this button to read the review contents.
A user with Reviewer rights can add review comments. If the Attending Doctor has their rights set as Mandatory, the encounter cannot be closed until it is reviewed.
The Review checkbox beside it is a provision to mark the encounter as reviewed without explicitly reviewing it.

Note: It is enabled only if the user is a Reviewer. Once the notes are added, the Notes button is disabled.

Create Claim: If this is checked, then on Save, a billing entry is created and is seen under Claims → Edit in Biller login.

Resource Appointment Encounter: A Resource Appointment encounter can be closed ONLY if the attending doctor is one of the eligible providers. The user is explicitly informed to change the Attending Provider with the message, "Please assign eligible Attending Provider to close the Encounter." Note that the Attending Doctor cannot be the Resource, and this information has to be edited or changed in order to close the encounter.

Mark Encounter Confidential: The encounter can be marked as Confidential by selecting this checkbox, in which case the details are not accessible to other doctors.

Mark Encounter Complete / Sign Off: To close the encounter, the user needs to select this checkbox and click on the Save button.
Note: The user can delete the empty templates associated with the encounter from this screen. On checking the Mark Encounter Complete / Sign Off checkbox and clicking the Save button displayed at the bottom of this screen, a warning message, "The following template(s) are blank. Select Yes to delete the blank templates and close the encounter, else select No to go back and fill the blank templates. Note: Associated CPT/HCPCS/ICD codes will also be deleted. " is displayed. On this warning message popup, if the Yes button is clicked, the blank templates identified by the system that are displayed on the warning message popup are deleted. Once the blank templates are deleted, along with the warning message, the encounter is also closed. If the No button is clicked, the blank templates are not deleted and the warning message popup is closed. The user is navigated back to the Encounter Close screen and the Mark Encounter Complete / Sign Off checkbox is unchecked.
It is important to note that this functionality takes place only for those Test Exec screens that are selected in the property enc.close.delete.blank.template. This property has the following options: VT - Vitals, RS - ROS, HP - HPI, SH - Social History, SC - Specialty, PH - Physical Exam.


Patient Declined Health Summary: Select this checkbox if the patient does not want the Health Summary.

Share Health Summary On Portal: Select this checkbox to share the patient's health summary on the Patient Portal.

Share Health Summary On API: Select this checkbox to share the patient's health summary with a third party via an exposed FHIR API (Fast Healthcare Interoperability Resources Application Programming Interface).

Referral Reason: Click on this button to enter the referral reason. The entered referral reason is saved and available in the Health Summary only after saving the Encounter Close screen.

Health
Summary: Click on this button to view the Health Summary.

Action Buttons:
Save: Click on the Save button to save the changes on the Encounter Close screen.

Reset: Click on the Reset button to reset the changes before the last save on the Encounter Close screen.

Delete: Click on the Delete button to delete an encounter.

Reopen: Click on the Reopen button to reopen a closed encounter.
