Topics Claims: Attorney Search:  

Introduction

User can enter the details of injury through this popup. This popup will enable the user to add the information.

Pri Ins Attorney: Click on the search icon to select a name of an attorney associated to selected Primary insurance. To reset the data, click on the 'Clear' link in the adjacent field.

Sec Ins Attorney: Click on the search icon to select a name of an attorney associated to selected Primary insurance. To reset the data, click on the 'Clear' link in the adjacent field.

Pri Ins Adjuster: Click on the search icon to select a name of an adjuster associated to selected Primary insurance. To reset the data, click on the 'Clear' link in the adjacent field.

Sec Ins Adjuster: Click on the search icon to select a name of an adjuster associated to selected Primary insurance. To reset the data, click on the 'Clear' link in the adjacent field

Date of injury: Enter the date of injury for the selected case or select it from a Calendar by clicking on the calendar icon. The user may enter a date in Standard mm-dd-yyyy Format. The date, if present on Patient Register > Case Management screen will be auto-populated in this field.

State of Accident: Select the name of the state from the drop-down menu list where an accident occurred. The state, if present on Patient Register > Case Management screen will be auto-populated in this field.

Unable to Work From: Enter the date from when the patient is unable to perform job duties on work for a period of time due to an injury or select it from a Calendar by clicking on the calendar icon. The user may enter a date in Standard Format mm-dd-yyyy. Unable to Work From date, if present on Patient Register > Case Management screen will be auto-populated in this field.

Unable to Work Up to: Enter the date till which the patient is unable to perform job duties on work for a period of time due to an injury or select it from a Calendar by clicking on the calendar icon. The user may enter a date in Standard Format mm-dd-yyyy. Unable to Work Up to date, if present on Patient Register > Case Management screen will be auto-populated in this field.
