Introduction: The Summary of Care facilitates the exporting of a patient's encounter details to a referral clinic provider. The exported file is in a ZIP format consisting of an XML file and an HTML file. The XML file (CCDA.xml) contains the patient's details, whereas the HTML is just the readable format of the XML file. The exported entry of the Summary of Care is displayed as a list in the CCD/CCR screen. Field Description From: Select a doctor's name from the Doctors pop-up invoked by clicking the search icon. To Dropdown: Select to whom the exported data is addressed from this dropdown. The available options are None, Provider, Patient, and Others. Patient: This field displays the patient's name. Type: Select the type of CCD that needs to be attached. The available options are Cures CCD 2.1, Cures Referral Note, CCD 2.1, Referral Note, CCD 1.1, CCD HTI 1, and HTI 1 Referral Note. Note: An additional option, Electronic Care Report 2.1, is displayed in the Type dropdown, depending on whether it is configured in the database. Person Details: First Name: Enter the first name of the person to whom the Summary of Care is being exported. Last Name: Enter the last name of the person to whom the Summary of Care is being exported. Add. Line 1: Enter the first line of the address of the person to whom the Summary of Care is being exported. Add Line 2: Enter the second line of the address of the person to whom the Summary of Care is being exported. City: Enter the name of the city where the person lives. Zip: Enter a valid ZIP code of the area where the person lives. Work Tel: Enter the person's work telephone number. Cell Phone: Enter the person's cell phone number. Fax: Enter a valid fax number for the person. E-Mail: Enter the person's valid email address. Clinic Details: Clinic Name: Enter the name of the referral clinic. Add. Line 1: Enter the first line of the address of the referral clinic. Add Line 2: Enter the second line of the address of the referral clinic. City: Enter the name of the city where the referral clinic is based. Zip: Enter a valid ZIP code of the area where the referral clinic is based. Work Tel: Enter the clinic's work telephone number. Fax: Enter a valid fax number for the referral clinic. Purpose: Enter the purpose for requiring the Summary of Care. Action Buttons Export Button: When clicked, the Export button starts the Summary of Care process for the selected patient. If the value of the property prognocis.filedownload.noencryption is turned Off, the encryption function is enabled and a User Password pop-up is invoked. Enter a password in this pop-up to use later to decrypt the downloaded ZIP file. If the property is set to On, no encryption procedure is involved and the ZIP file is downloaded directly. The downloaded ZIP file is named with the patient's name and an '.exe' file extension. Note: ECR CCDs can be exported only when the system-level property ecr.ccd.export.soc is turned On. Reset Button: When clicked, the Reset button resets the fields to blank.
