| It is assumed that when a new patient comes in, the
doctor or nurse collects information on the patient's past surgeries and procedures. Delete: Select this checkbox to delete the row on Save. When: A non-mandatory text field for the date of surgery (an approximate date is OK). The user can enter Jan 2001. If they know the exact date, they can click the Date icon to invoke the calendar and select a specific date. Procedure: A short note on the procedure performed. Hospital: The name of the hospital where the surgery was performed. Doctor: The name of the surgeon who performed the procedure. Relevance: Relevance to the present condition. If the practice is an endoscopy clinic, a surgery involving a fracture of the leg might not be relevant. The user can flag it accordingly. The list is always sorted by Relevance followed by When. The records with Relevance checked are highlighted in a different color on the Facesheet. Notes: Add notes with phrases to the surgical information. You can do this by using an acronym. The favorite acronyms are shown in the autocomplete search. Select any acronym or type the acronym and press the Tab key to populate the Past Surgical History Notes.
