Topics Patient Referral Tracking Search:  
Introduction: The Referral Management system consists of two parts: Referral Add/Edit and Referral Tracking.
Referral Tracking enables the user to track referred patient(s) status(es) which are added in the 'Referral Add/Edit' popup. The data displayed in tabular form also helps the user to track the work done by a particular 'Referral Coordinator'.



Field Descriptions:

Filters Section

All: Check/Select this field and click the Go icon: , then the Referral Tracking output is generated displaying details of all referrals irrespective of status.

Referral Accepted: Check/Select this field and on clicking the Go icon: , the Referral Tracking output is generated displaying the details of Accepted Referrals.

Referral Pending: Check/Select this field and on clicking the Go icon:  , the Referral Pending output is generated displaying the details of Pending Referrals.

Referral Rejected: Check/Select this field and on clicking the Go icon: , the Referral Rejected output is generated displaying the details of Rejected Referrals.

Referral Coordinator: Select the name of the Referral Coordinator whose referrals are to be tracked. Only active users assigned with the User Role 'ReferralCoordinator' are displayed in this drop-down list.

Referral Status: Select a filter for the type of Referral status from this drop-down list. Values from the Referral Status drop-down of the Referral Add/Edit popup are fetched and displayed in this drop-down list.

Contact: Select a filter for the type of follow-up used for Referral tracking. There are two default options in this drop-down list, as only the referred Patient and his/her corresponding Insurance can be tracked through Patient Referral Management.

Attempt: Select an action performed filter for the referrals from this drop-down list. Values from the Contact Action drop-down of the Referral Add/Edit popup are fetched and displayed in this drop-down list.

Follow-Up Action: Select a filter for the response received through the action performed for the referrals. Values from the Response drop-down of the Referral Add/Edit popup are fetched and displayed in this drop-down list.

Patient: To view referral data associated with a particular Patient, select the respective Patient's name from the available list. This list only displays Patient names added from the Referral Add/Edit popup (both 'Active' and 'Referral' statuses) and is arranged in alphabetical order of the Last and First Name respectively.

Referring Provider: To view referral data associated with a particular Referring Provider, select the respective Referring Provider's name from the available list. This list shows all 'Active' Referring Providers in alphabetical order of the Last and First Name respectively.

Referral Date: To view results based on the Referral Date set in the Referral Details section (the date set while adding the respective referral to the system) of the Referral Add/Edit popup, select this radio button. This option is selected by default.

Contacted Date: To view results based on the date set in the Referral Follow-up section (the date set while adding a new Contact Action taken to the system) of the Referral Add/Edit popup, select this radio button.

Period: The user can select the desired period from this drop-down list. Once the Period is selected, the related dates are auto-populated in the From and UpTo date fields, except for Custom Date. When the Custom Date option is selected, the user is required to manually select the dates in the From and UpTo date fields.

Referral Tracking Results Section

The following columns are displayed in the Referral Tracking Results output section:
    • Patient Name
    • Type
    • Attempt
    • Referral/Contacted Date
    • Follow-Up Action
    • Referral Status
    • Comments
    • Referral Coordinator
    • Create Date
    • Date of Birth
    • Referring Physician
• Referral Tracking details are populated in the aforementioned columns of the Results section based on the filters selected in the Filters section above. Tracking results are shown for patients having both 'Referral' and 'Active' statuses.
Note: Only tracking results of patients with the 'Referral' status and that are marked as 'Active' from the 'Referral Add/Edit' screen are considered for display. Patients entered directly from the Patient → Register screen or from CSV Import are not displayed in this screen's results section.

• On selecting filters and clicking the Go button, the patients' tracking records are displayed in descending order of 'Referral/Contacted Date'. If the Referral/Contacted date is the same, then these results are further sorted (secondary sort) based on the patient's Display Name.

• On clicking the Patient Name hyperlink (e.g., Dummy - the name displayed under the Patient Name column in the results section), the user is redirected to the Referral - Edit popup in order to take action or view details depending on the patient's status (Referral/Active).

• Patient information on the Referral-Edit popup can be edited ONLY for patients having their status set as Referral. For patients having the status Active (marked as Active from the Referral Add/Edit popup), the Referral-Edit popup displays the respective patient's information but does not provide editing rights.

• Updates made to the patient entries with 'Referral' status will not be reflected automatically on the Referral Tracking screen. The user has to explicitly click the Go button to view the updated data.


Other Buttons / Icons

Pagination: The Referral Tracking Results section displays ONLY 30 patient entries on one page. If the search criteria have more than 30 patient entries to be displayed, then pagination appears in the bottom right corner of the Results section. This pagination has First, Previous, Next, and Last buttons, as well as an editable text box to enter the desired page number along with the total number of pages (number count label).

Print icon : By clicking this icon, the user can convert tracking reports to PDF format and further print this PDF from a connected printer.

Export XLS icon : By clicking this icon, the user can download the referral tracking reports in XLS format to their local system.

Properties: This button is exclusively visible to users with Read Access to the Properties Master screen. Access to this button is strategically granted to enhance user functionality. Clicking the Properties button triggers a popup, presenting a list of properties from the Properties Master screen in PrognoCIS that cater to the Referral Tracking feature.
