Topics Patient Referral Tracking Search:  
Introduction: Referral Management system consists of two parts; Referral Add/Edit and Referral Tracking.
Referral Tracking enables User to track referred patient(s) status(es) which are added in 'Referral Add/Edit' popup. Data displayed in tabular form also helps User to track the work done by a particular 'Referral Coordinator'.



Field Description :

Filters section

All: Check/Select this field and click on GO icon: , then Referral Tracking output gets generated displaying details of all the Referrals irrespective of the status.

Referral Accepted: Check/Select this field and on clicking GO icon: , Referral Tracking output gets generated displaying the details of Accepted Referrals.

Referral Pending: Check/Select this field and on clicking GO icon:  , Referral Pending output gets generated displaying the details of Pending Referrals.

Referral Rejected: Check/Select this field and on clicking GO icon: , Referral Rejected output gets generated displaying the details of Rejected Referrals.

Referral Coordinator: Select name of the Referral Coordinator whose referrals are to be tracked. Only active Users assigned with the User Role 'ReferralCoordinator' are displayed in this drop-down list.

Referral Status: Select filter for the type of Referral status from this drop-down list. Values from Referral Status drop-down of Referral Add/Edit popup are fetched and displayed in this drop-down list.

Contact: Select filter for the Type of Follow-up used for Referral tracking. There are two default options in this dropdown list as only the referred Patient and his/her corresponding Insurance can be tracked through Patient Referral Management.

Attempt: Select action performed filter for the Referrals from this drop-down list. Values from Contact Action drop-down of Referral Add/Edit popup are fetched and displayed in this drop-down list.

Follow Up Action: Select filter for the response received through action performed for the Referrals. Values from Response drop-down of Referral Add/Edit popup are fetched and displayed in this drop-down list.

Patient: To view referral data associated to a parcticular Patient, select respective Patient's name from available list. This list only displays Patient Names added from Referral Add/Edit popup (both 'Active' as well as 'Referral' Status) and is arranged in alphabetic order of Last and First Name respectively.

Referring Provider: To view referral data associated to a parcticular Referring Provider, select respective Referring Provider's name from available list. This list shows all 'Active' Referring Providers in alphabetic order of Last and First Name respectively.

Referral Date: To view results based on Referral Date set in Referral Details section (date set while adding respective Referral to the system) of Referral Add/Edit popup select this radio button. This option is selected by default.

Contacted Date: To view results based on date set in Referral Follow-up section (date set while adding a new Contact Action taken to the system) of Referral Add/Edit popup select this radio button.

Period: User can select desired period from this dropdown. As the Period is selected, related dates are auto-populated in From and UpTo date fields, except for the Custom Date. When the option Custom Date is selected then User is required to manually select the dates in From and UpTo date fields.

Referral Tracking Results section

Following columns are displayed in Referral Tracking Results output section:
    • Patient Name
    • Type
    • Attempt
    • Referral/Contacted Date
    • Follow Up Action
    • Referral Status
    • Comments
    • Referral Coordinator
    • Create Date
    • Date of Birth
    • Referring Physician
• Referral Tracking details are populated in aforementioned columns of Results section based on the filters selected in above Filters section. Tracking results are shown for patients having status, Referral and Active both.
Note: Only tracking results of Patients with status 'Referral' and which are marked as 'Active' from ‘Referral Add/Edit’ screen are considered for displaying. Patients entered directly from Patient → Register screen or from CSV Import are not displayed in this screens result section.

• On selecting filters and clicking Go button, patients tracking records are displayed in descending order of 'Referral/Contacted Date'. If Referral/Contacted date is same then further these results are sorted (2nd sort) based on Patient’s Display Name.

• On clicking Patient Name hyperlink (e.g. Dummy - Name displayed under Patient Name column in results section) User is redirected to Referral – Edit popup in order to take action or view details depending on Patient’s status (Referral/Active).

• Patient information on the Referral-Edit pop-up can be edited ONLY for Patient’s having status set as Referral. Patients having status Active (marked as Active from Referral Add/Edit popup) displays respective Patient’s information in Referral-Edit popup but does not provide its editing rights.

• Updates done on the patient entries with 'Referral' Status will not reflect automatically on the Referral Tracking screen. User has to explicitly click on GO button to view updated data.


Other Buttons / Icons

Pagination: The Referral Tracking Results section displays ONLY 30 Patient entries on one page. If the search criterion has more than 30 Patient entries to be displayed, then pagination appears in the bottom right corner of Results section. This pagination has First, Previous, Next and Last buttons and also an editable textbox to enter desired page number along with total number of pages (number count label).

Print icon : Clicking this icon User can convert tracking reports in PDF format and further take a print of this PDF from a connected Printer.

Export XLS icon : Clicking this icon User can download the referral tracking reports in XLS format into their local system.
