Introduction:
When a new Patient comes in, the doctor or nurse collects information on the Family History of serious congenital problems. The Family History screen is prepopulated with the doctor's preferred family history problem. The user can add to the list by entering details on a blank row -
the added problem or relation is specific to the patient and remains editable.
The user can set the property, facesheet.familyhistory.byrelation (at the Admin Level) to decide if they prefer the Family History
screen to be displayed as Problem + Relation OR Relation + Problem on the Face Sheet details screen.
In other words, depending upon the property, facesheet.familyhistory.byrelation , the List of Problems OR Relation is displayed in the right pane.
Upon selecting a single row in the center pane, multiple options to
select the Relation from the right-hand pane are available. Clicking the Add
button generates as many rows as the number of options selected.
The associated SNOMED codes for the list in the center pane have the codes displayed on their tooltips.
The Problem [+] search displays the associated SNOMED code in a separate column; there is a provision to enter Notes too.
Details in the Face Sheet cell will also be displayed accordingly (including Notes).
Sel: Check this to
select the row upon
clicking the OK button.
Problem: A problem can be added by clicking the
Add Problem button. The corresponding Relation(s) from the list on the right-hand side can be selected and the OK button clicked to populate the new rows. More problems can be added by clicking the
button.
Note:
The manually entered Relation and/or Problem will always be displayed as enabled, even after saving the details.
SNOMED [+]: Click the
SNOMED
button to add Family History Problems based on SNOMED codes. The maximum number of Family History SNOMED codes that can be selected from the IMO search for adding to an Encounter is dependent on the value set in the property,
newrows.familyhistory. Once the number of added SNOMED codes reaches the value set in the property,
then an alert is displayed on the screen as 'Select Max No of
Codes', which restricts the user from adding more records.
After adding the SNOMED codes, the tooltip of the added Problem column displays the description of the Family History SNOMED code and its associated SNOMED code.
Relation: Enter the
Relation for the selected Problem or select a Relation from the list
available on the right-hand side of the screen. After selecting the Relation,
click the Add button to display the selected Relation for the respective
Problem. If the user selects the Relation first and clicks the Add
button without selecting the Problem, then a notification is displayed on
the screen as 'Please add Problem first'.
Note:
Once the number of added SNOMED codes reaches the value set in the property,
newrows.familyhistory
and the property facesheet.familyhistory.byrelation is turned ON, then the respective Relation [+] or SNOMED [+] button is displayed as disabled, thereby restricting the user from adding more records.
Comments: Any other comments to be made by the doctor.
Relevant: Relevance to the present condition. The user can flag the mentioned family history of the patient if it is relevant; the records with Relevance checked are highlighted in a different color
on the Face Sheet.
Hint:
Irrespective of the property, facesheet.familyhistory.byrelation being turned ON or OFF, it is always recommended to select the value in the first column and then associate the respective information in the second column.
Notes Textbox Area: Enter the Notes for the Family History Problem
as required. You can do this by using an acronym. The favorite acronyms are shown in the autocomplete search. Select any acronym or type the acronym and press the Tab key to populate the Family History Notes. The character limit is up to 5,000 characters.
Zoom Icon: Click the Zoom icon:
, which invokes the Family History Notes pop-up, where the Insert Phrases icon:
is displayed.
Clicking the Insert Phrases icon
invokes the Fast Phrases pop-up, from where the user can select or add phrases as required.
Clicking the Save button on the Fast Phrases pop-up lists the added phrases
under the Family History Phrases pop-up.
Insert Phrases Icon: Clicking the Insert Phrases icon:
invokes the Fast Phrases pop-up. The phrases for Family History are displayed on this pop-up, from where the user can add new phrases or select existing phrases as required.
Once the user clicks the Save button on the Fast Phrases pop-up, the
added phrases are listed under the Notes section of the Family History screen.
OK Button: Clicking the OK button saves the selected Family History problem or relation.
Cancel Button:Clicking the Cancel button directs the user to the Face Sheet screen.
History Button:
Clicking the History button invokes the Family History list pop-up,
which displays the list of selected Family History problems and
relations along with their SNOMED relations and problems information. Additionally,
the comments and the dates when the Family History problems were added in
PrognoCIS are displayed under the Comments and Date columns.
Print Button:
Clicking the Print button allows the user to print the Family
History problems.
