Note that the amount paid by the insurance company is dictated by the schedule which they use, NOT by the amount billed. So, although the clinic might bill a premium for a senior provider, the insurance company is still likely to pay the same. In the event the insurance company is prepared to consider a deviation for a specific provider, the user can define the schedule names and percentages applicable for different insurance companies, to be used for this provider.

Insurance Name:Search and select the insurance company name.

Schedule: Select the schedule to be applied.

Percent: Set the percentage rate.
