Introduction: The Health Summary, also known as the Clinical Summary or MU Summary, is available in the standard CCD format in PrognoCIS. This Summary is generated from data entered in a patient's current encounter (a summary of data such as the Problem List, Current Medication, PMH, etc., is included). A clinic's MU count increments if a patient's Clinical Summary/Health Summary is provided to the patient or the patient's authorized representatives (in printed or downloaded form) within one business day from the encounter's start date.

PrognoCIS supports the export and import of the Cures 2.1 Continuity of Care Document (CCD) and the Cures 2.1 Referral Note. Users can view and download this version of the CCD and Referral Note from the following paths:
  • Patient → Encounter → Encounter TOC → Encounter Close → Click the Health Summary button
  • Patient → Encounter → Encounter TOC → Summary of Care
  • Patient → Review → Review TOC → Summary of Care
  • Patient → Letters Out → Click the Attach... button → Select a New Continuity of Care Document (CCD)
  • Patient → Letters Out → Click the Attach... button → Select a Referral Note (CCD)
  • Patient Portal → My Health Record → API Access (FHIR) → Encounter → Click the View CCD button
The downloaded files are exported in the form of consolidated PDF and CCD files (XML and HTML files) in an encrypted ZIP format. The XML file contains the patient's details, and the HTML file is just the readable format of the XML file.

A Health Summary record, also known as a Summary of Care record, consists of the following elements:
  • A message "Total Sections to be sent: 28" is displayed in the top-right corner of the Summary. The number 28 corresponds to the total number of sections present in the Summary.
  • The Patient Information section displays patient details such as patient name, Date of Birth, Sex, Race, Preferred language, Ethnicity, contact information (including telephone numbers and email addresses), patient IDs, Document ID, and the date of document creation.
  • The Show Clinic Contacts button; clicking this button expands the Clinic Contact Information section and displays Performer (Primary Care Physician or Nurse Assistant) information, Author Contact Information, Informant Contact Information, Legal Authenticator Contact Information, and Document Maintained By Contact Information. The Hide Clinic Contacts button appears when the Show Clinic Contacts button is clicked; clicking this button collapses and hides the Clinic Contact Information section.
  • A Table of Contents (TOC), which consists of a list of sections that display Clinical Notes data from various screens of PrognoCIS. All the sections in the TOC are displayed as hyperlinks, which, when clicked, navigate to their respective sections. Clicking the up-arrow button collapses the TOC section, whereas clicking the down-arrow button expands it. By default, it is displayed in an expanded format.

    Note:
    • The property ccd.show.sections is used to decide which sections must be displayed in the TOC in a CCD or Referral Note by default.
    • The sections set in the property ccd.show.sections should also be set in the property ccd.show.expand, which is used to display the sections as expanded by default.

    A checkbox is displayed to the left of each section in the TOC. Only those sections for which the checkboxes are selected are displayed in the CCD. The selected sections are displayed in the same sequence as entered in the property. If the All checkbox is selected, then all the sections will be displayed.
  • The Expand All / Collapse All button, which, when clicked, expands or collapses all the sections in the CCD. If there are any sections already in an expanded format, then clicking the button collapses those sections, and vice versa. Individual sections can also be expanded or collapsed by clicking the up or down arrow buttons provided on the section band next to the section name in the CCD.

    The sequence of sections in the CCD can be changed by clicking on the section band and using drag-and-drop functionality.

    Note:
    • Changing the sequence of sections in the CCD would not affect the section's sequence in the Table of Contents.
    • The changed sequence is for viewing purposes only; thus, it is reset upon closing the CCD.
Action Buttons:

Recreate Button: Click this button to recreate a fresh Health Summary from that point in time to the latest available encounter data. This button is disabled if the Summary has already been printed or downloaded.

Edit Button: Click this button to edit the Notes content available in the Health Summary, delete section content, or delete an entire section. Upon clicking this button, a popup with the message "Select the checkbox to exclude a section from the Summary" is displayed along with the Save and Cancel action buttons. If no changes are made, then clicking the Cancel button closes this popup, and the user is navigated back to the Health Summary popup. If any changes are made in the Summary, they are saved upon clicking the Save button, and a checkmark is displayed on the Edit button on the Health Summary popup.

Print Button: Click this button to print the Health Summary. Upon clicking this button, the system sends the PDF of the Summary to the default printer for printing, and a checkmark is displayed on the Print button on the Health Summary popup.

Download Button: Click this button to download the Health Summary. If the encryption property prognocis.filedownload.noencryption is On, then the content is downloaded in an encrypted ZIP format consisting of XML and HTML content; otherwise, a non-encrypted ZIP file consisting of XML and HTML content is downloaded. A checkmark is displayed on the Download button on the Health Summary popup after a successful download of the Summary.

Close Button: Click this button to close the Health Summary popup.

Note: Each time a Health Summary is either printed or exported, an entry is registered on the CCD/CCR screen found on the Review tab (Patient → Encounter → Review → Tab).
