The Address popup allows users to enter additional personal information such as a postal contact address, telephone numbers, email addresses, etc. This popup is common to various PrognoCIS screens since there is a need for address information of various entities such as:
  • Doctor/Provider
  • Nurse
  • Staff
  • Referring Doctor
  • Laboratory
  • Radiologist
  • Pharmacy
  • Patient
  • Patient's Emergency Contact
  • Patient's Employer
  • Patient's Spouse / Legal Guardian
Field Descriptions:

Address: Enter the contact address. This field usually contains 2 lines, i.e., Address Line 1 and Line 2. The character limit of these two lines is 35 characters. If Address Line 1 is entered, then City also has to be entered and vice versa.

ZIP: Enter a ZIP code. If the selected Country is the USA and a valid ZIP code is entered, then City and State are automatically detected and displayed. ZIP can be set as mandatory if required by turning On the properties zip.code.mandatory and zip2.code.mandatory from the Properties Master screen. An edit mask can also be set for ZIP by turning On the properties zip.mask.applicable and zip2.mask.applicable.

City: Enter the name of the city of residence.

State: Enter the name of the state of residence. While adding a new address, the state in the Clinic Address is taken as the default.

Country: Enter the name of the country of residence. USA is selected as the default option here. If the user lives outside the US, select the Non US checkbox and enter the name of the country.

ISD Prefix: Enter the International Subscriber Dialing (ISD) Prefix of the country. This field is enabled only if the Non US checkbox is selected.

Cell Phone: Enter the cell phone number.

Work Tel.1: Enter the work telephone number.

Work Tel.2: Enter an alternate work telephone number.

Note: The maximum number of characters accepted in the Cell Phone, Work Tel.1, and Work Tel.2 fields is 20 characters. However, it is dependent on the format set in the properties tel.mask.applicable and tel.mask.format, as follows:
  • If the property tel.mask.applicable is turned On from the Properties Master screen, then masking (as per the property tel.mask.format) is applicable to the number. If the property is turned Off, then the number is displayed in the default format.
  • The property tel.mask.format defines the mask style of phone numbers in PrognoCIS. For example, if the value in this property is 999 999-9999 x 999, then the number entered in the field is displayed in the same format as the value entered; otherwise, it is displayed in the default format.
Fax: Enter the fax number.

Email: Enter the email address of the user. This field has a character limit of 50 characters. Although this field is not mandatory, it is advisable to enter an email address here as the user ID and password generated for the user after registration are sent to their registered email address.
 
Secure Email: Enter a secure N2N email address for the user. The PrognoCIS Clinical Messaging (N2N) feature allows Providers to electronically exchange clinical messages across disparate healthcare systems in a secure manner. Thus, there are many external healthcare entities (EMRs, pharmacies, PBMs, etc.) that send N2N messages directly to a Provider's or Clinic's inbox.
  • For Providers, an N2N email must be set in the Secure Email field on SettingsConfigurationUsers column → ProviderAddress button → Address for <Provider> popup (not the Billing Address button). After the N2N email is saved successfully and Surescripts eRx registration is done via the eRx button, incoming N2N messages will be sent to their MessagesInbox screen with the subject prefix: Clinical Msg. Providers can send N2N messages from the PatientLetters Out screen.
  • For Clinics, an N2N email must be set in the Secure Email field for the clinic's location in SettingsConfigurationClinic column → LocationAddress button → Address for <Location> popup (not the Billing Address button), and a receiver must be added in the Secured Email Location Support field on the Location Master. After the N2N email is saved successfully and Surescripts eRx registration is done via the eRx button, all clinic's location-level N2N email messages will be sent to this receiver's inbox.
  • For Referring Providers, the Secure Email field is provided only for the purpose of quickly setting up the recipient Referring Provider's N2N email while sending a secure N2N message from PatientLetters Out. A Referring Provider does not have access to the PrognoCIS EMR Inbox. If a Referring Provider is imported into PrognoCIS via the Import button, the Secure Email field becomes disabled after import.
NCPDP ID: This field is specific to the Pharmacy Vendor Address. The Pharmacy NCPDP ID is available in this display-only, non-editable field.

Pharmacy NPI: This field is specific to the Pharmacy Vendor Address. The Pharmacy NPI is available in this display-only, non-editable field.

Note:
  • The NCPDP ID and Pharmacy NPI fields show details for pharmacies listed in the pharmacy database received from Surescripts or imported from the Surescripts pharmacy database.
  • Pharmacies added manually are excluded from this information.
  • If these conditions are not fulfilled, these fields remain blank.

From Date: Indicates the start date of the patient's stay at the previous address.

Upto Date: Indicates the end date of the patient's stay at the previous address.

Action Buttons:

OK button: Click this button to save the details added on this popup and close the popup.

Cancel button: Click this button to discard the details added on this popup and close the popup.
