What is PrognoCIS Patient Portal?

PrognoCIS Patient Portal is a secure web application that connects you with the clinic and presents to you information about your health care and treatment.

Why should I sign up?

Signing up with PrognoCIS Patient Portal is a great way to connect with your clinic and Providers that assist you in your healthcare and treatment. It gives you an insight on your health information available with the clinic. You can also connect with the clinic securely from the comfort of your home/office as you can access the Patient Portal where and whenever you need it.

What can I see and do?

On logging into PrognoCIS Patient Portal, you can see your health care summary, details of existing problems and diagnocis, treatment provided, upcoming appointments, previous clinic visits, lab/rad test results. You can schedule appointments, request for prescription refill, send and receive secure messages from clinic, make payments conveniently.

Can I see my Provider details on the portal?

Yes, you can see the your Primary Care Provider details on the portal. The Provider’s image, name and contact information is shared with you.

Registration

Can I self-register using PrognoCIS Patient Portal?

Yes, self-registration is possible using PrognoCIS Patient Portal. Just click on the Sign Up button on the homepage, it will direct you to the self-registration form. You can quickly enter you information and register with the clinic.

Forgot Login Details

What should I do if I have forgotten my portal’s password?

You can reset your login password from the Portal’s Login page by clicking on the Forgot Login Details link. You will be required to enter and submit your User ID and your Cell Phone number under the Forgot Password tab to reset your password.

What should I do if I have forgotten my login credentials?

You can retrieve you login username and password from the Portal’s Login page by clicking on the Forgot Login Details link. You will be required to enter your First Name, Last Name, Date of Birth, Email Address and your Cell Phone number under the Forgot Login Details tab.

Your Information

Where does my information come from on the Patient Portal?

Information that is visible on the Patient Portal is fetched directly from your medical records that is available with the clinic in the clinic’s electronic health data recording system.

How can I update my personal information including my home address, email or change my password?

You are required to log into the Patient Portal, then locate the Profile icon and menu on the top panel, select Basic Details option from the Personal Info menu, you will be directed to My Profile page. You can then edit or add your personal information.

Can I change my information from Patient Portal if it is incorrect?

It is recommended that you connect with the clinic to get your records updated in case you find that the information displayed on the Portal is either incomplete or incorrect. The best way to connect with the clinic is using the Message Center feature available on the portal.

Why is some of my information not shown in the Portal?

Some information may require the clinic’s intervention before making it available on the Portal.

When can I see my test results in patient portal?

Your test results are available on your patient portal account after the clinic has reviewed them. Test reports are generally available within 1-7 business days.

When can I expect a reply if I send a message to my Primary Care Provider in the clinic?

You will generally receive an answer within 1-3 business days. For any urgent medical needs, it is recommended that your contact the clinic directly instead of sending a message from the portal. In case of medical emergencies, please call 911.

Documentation

How can I share my reports or additional information with the clinic?

You can use the Patient Portal’s Attach Document feature to share your reports or additional information with the clinic. This feature lets you attach and upload images and documents. You can also share a reference link for your documents if they are available on the internet.

Can I download and/or print documents from my Patient Portal account?

Yes, some documents on the Patient Portal can be downloaded or printed. To print a document, make sure you have access to a printer, you may then download and print documents from the Patient Portal account.

Appointments

Can I schedule, change or cancel appointments using the Patient Portal?

You can schedule or request an appointment from the Patient Portal. However, at this point of time, cancelling and rescheduling of the appointment is not possible. In this case, you may contact the clinic or use the Message Center feature of the portal to intimate the clinic if you want to cancel or reschedule an appointment.

Prescription

Some of my medications are missing from the list on the Patient Portal?

You will only see medications that are prescribed and are in effect. The discontinued medications will not appear on the Patient Portal.

Access, Privacy, and Security

How do I access my Patient Portal?

If you have already signed up for an account, you can login using the Patient Portal URL provided to you or download the My Health Records App onto your mobile device from the App store or Google Play store.

Who can access my account?

No one other than you can access your Patient Portal account without your consent. You will get a unique username and password to access Patient Portal when you sign up. It is recommended that this information should not be shared.

Can I give others access to my Patient Portal account?

Yes, by sharing your login username and password, you can consent for another person to access your account. Instead of sharing your login credentials, you might want to create an Authorized User account for the person who can access some parts of your account. Please note that not all information is available to the Authorized User.

What is an Authorized User?

A person in your care can be associated with your Patient Portal account. This person is known as Authorized User. Separate login credentials are allotted to such a user. Only limited/necessary information is shown to the Authorized User.

Who can help me in case I am unable to access my Patient Portal account?

Contact our help desk or email us if you are unable to access your Patient Portal account.

Is my information secure?

PrognoCIS, powered by Bizmatics is dedicated to protecting your privacy and confidentiality. Our system uses the latest security technology and we make sure that we never send you emails or texts asking for your unique login credentials. In case, you receive a suspicious email or text enquiring about your account, please call the clinic or email us to confirm if the email or text is from us.
