Topics Allergies Search:  

Introduction: Usually, when a new patient comes in, the doctor or nurse collect information on all his allergies or intolerances. This information helps to minimize chances of adverse reactions in prescription. The Allergy screen has two parts, Allergy Section (on top) followed by Transition of Care section

Allergy Section
Existing and All : drop-down displays the list of allergies that are either currently existing for the Patient or the entire list applicable for the selected patient.

No Known Drug Allergies: The option when selected marks that the patient has no known drug allergies. Once a Drug Allergy is selected, this Check Box is unchecked and not available for clicking.

Transition of Care section: The following field description is equally applicable to the Allergy section

Add Allergies: The drop-down categorises the allergies as Environmental, Food, and All

To enter allergy: Click to select the allergy from the search Pop-Up which has the consolidated list of all the above allergen categories.
Check the allergy from the list to select it.

No Known Transition of Care Medication Allergies: The option when selected marks the encounter as no reconciliation done for medication allergy in the encounter.

Del:
The option when selected on a particular row would delete that particular row of details, On save.

Type: Allergies are classified into six types:

1) Environmental User enters allergy description such as Pollen, Dust.
2) Food User enters allergy description, such as Chocolate.
3) Generic Drug Generic / Brand Name.
4) Dispensable Drug This is a Generic + Dosage details. A patient can be allergic to a higher dosage, but not to a lower dosage.
5) Allergens Drug Allergen Groups.
6) Cross Sensitivity To Ingredients.

Name: Select Allergy Description from the drop-down list, the selected value is displayed as allergy type under the Type column. If the allergy is selected from the plus button [+] then the Name field is populated with the selected allergy name. However, you can still edit the name if required.

Reaction: Enter a brief description of the adverse reaction caused.

Allergy / Intolerance: Select Allergy or Intolerance from the list box. A patient may be intolerant to iron tablets causing constipation. If patients have Intolerance to certain drugs, they can be warned or given supplementary drugs. Allergy is a life threatening reaction.

Created On: The creation date is applicable for allergy details coming in from CCD-CCR.

Status: The possible Status values are:
  1. Existing
  2. Inactive
  3. Erroneous
Source: The drop-down list displays various sources from where the drug has been added. It is a mandatory field.

The allergy imported through CCD file.

PL

The allergy added through ‘+' button from ‘Transition of Care' section and selected with Value as PL (Patient List) AND marked for old Allergy added Data.

OldAllergy

The allergy added through ‘+' button from ‘Transition of Care' section and selected with Value as OLDAllergy.

RD

The allergy added through ‘+' button from ‘Transition of Care' section and selected with Value as RD (Referred by doc).


Reconciliation: The drop-down list by default displays value, ‘Add' for problems added via Ailments [+]/ICD [+] whereas it displays value, ‘Select' for problems imported via CCD.

Note:

Transition of Care section consists of allergies that have been entered via the [+] button, and/or allergies imported through CCD files. If the CCD file has been imported in an encounter of the patient then the details of these imported allergies are displayed on the Transition of Care section from the next encounter onwards.
The Del checkbox is disabled for the allergies imported through CCD.
The Reconciliation drop-down list by default displays value, ‘Add' for allergies added via [+] whereas it displays value, ‘Select' for drugs imported via CCD.
To add allergy to the patient's current allergy list from Transition of Care section, User has to select the Reconciliation action as ‘Add' on the allergy. To remove allergy from Transition of Care section, User has to select the Reconciliation action as ‘Remove' – this entry is not added to the patient's allergy list.
Once an allergy has been reconciled then that entry is removed from Transition of Care section.
Duplicate Allergies are decided upon the Type and code of Allergy.

Tthe system checks for the Allergens / Cross Sensitivity / Generic Drug / Dispensable Drug conditions at the time of Prescription to ensure that Patient is not allergic to any of the prescribed drugs.

Last Modified: The field displays information such as the latest modified date, time, and time zone of clinic location.
Notes: : Add notes with phrases to the patient's allergy information. The tag, ENC_FSS_ALLERGIES__NOTES displays the Allergy Notes information on the Progress Note
save: On click saves the details.
close: On click closes the Allergy screen.
history: On click 'Allergy History' popup is displayed, displaying the trace or log history of allergies added and/or modified by User.
print: On click prints the clinic details as header followed by the patient details and the patient's Allergy details.
