A Template is a Set of Tests. The Doctor can create
Any number of templates in each of the following:
- VT: Vitals
- HP: History of Present Illness
- SH: Social History
- RS: Review of Systems
- PH: Physical Exam
- SC: Specialty
For Example, in case of Physical Exam, the set of tests to be executed
will be different for Adult Male, Adult Female, Children, and Very Old
people. He can create a Template for each, with the appropriate set of
Tests. During an Encounter he can select the right Template.
The system displays all the lists of tests in the Database grouped Class
wise whose Group Number is "0". For the template just check
the required Tests. Please note that only Tests under Group No. zero are
displayed here. When Results for any one of these tests are entered and
sub-question set is applicable, they will get included at run time.
Templates are of Two Types. System Template and Doctor Template. Note
the Subtle Difference between the two. When a Administrator creates them
in option Settings \ Configuration \ Test Templates , they are flagged
as System Templates. These will typically be common for the whole clinic.
If the option is invoked by choosing Settings \ My Preferences \ Templates
they are flagged as logged in Doctor specific Templates.
When Any Doctor needs to select a Template during an Encounter, he is
Shown a list of Templates created by him, followed by the System Templates.
He can then select anyone of them.
For Specialty and Clinic Forms, a Form Template for printing can be associated.
Copy Template: Displays a list of Clinic preferred templates to choose and copy from.
In case of Specialty Template
User can select multiple associated CPTs by selecting them from the search button provided next to CPT text field.
User can select multiple associated HCPCs.
A copy of an existing Template can be created using the "save
as..." button.
Export button: User enters a filename
explicitly. The program then creates a XML file. This will contain details
of all the Tests selected in Template. All the fields for each of the
selected Test will be present in the XML. The XML Tags for the Test Fields
will be same as the Database Field Names. Note that the Template displays
Only the list of Tests which have Group No 0. A Test could Zoom to say
Group No 100, 200. Each of the question under Group 100 or 200 could further
zoom to other groups. The program finds all such questions and also includes
them in the exported XML file.
Import button: The program selects
a file using the Browser from the client machine, and tries to Import
the XML into the current Template option. Note that if is possible to
export a ROS template and import it into PH. In short Template Type used
will be Current, Not from XML file. The Category / System / Class code
read from XML if Not present will be added to the Database. The Test Code
if Not present will be added to the Database. In case of Duplicate tests
select an option
a) Ignore this XML test
b) Update the Test with Current contents
c) Add the Test with new Test Code.
Grid Tests Definition:
Some clinics require the ability to enter multiple results for the same test in the same encounter. This can be done in the test executions for Vitals, Social History, HPI, Review of Systems, Physical and Specialty.
To facilitate this, user can define a Grid in the Templates definition of all the above tests in Settings ' Configurations ' Tests by,
Select System: Select the system by clicking on the search button.
Occurrences: Upto 20 number of occurrences can be selected from the List Box to be entered here, but note that if this is set as zero, then the "Grid Tests.." button is in disabled mode, on the respective test execution forms.
Grid Test as: The Grid can be displayed with Test along Rows and Occurrences as Columns, or vice versa. Choose the radio button for Row or Column accordingly.
Name [+]: Click [+] to invoke CPT/HCPC popup to choose codes (CPT/HCPC/SNOMED) to be assigned.
It is mandatory to specify at least one of the three names given for the disease. The user name will be used to specify this CPT/HCPC code in PrognoCIS. Since the names are imported from the External CPT/HCPC database the user is allowed to change the name according to User's nomenclature.
CPT/HCPC Search popup:

The search is functional for CPT/HCPC and SNOMED codes.
Code/Description: Select this option and either enter a CPT code or the code description directly in the 'Search' field to search for results.
SNOMED: Select this option to enter a SNOMED code directly in the 'Search' field to search for results. Note: SNOMED search does not support description based search.
Search : Click 'Search' button to display the filtered list based on the option selected.
Selected: The field displays the codes that are selected by the User from the search result.
Ok: Click ok to close the Search popup. |

Following is the column details of the list:
Code : This is the CPT/HCPC or the SNOMED code.
Type : It displays the code name i.e. either CPT or HCPC or SNOMED
Description : It displays the code description
SNOMED Code : It displays the SNOMED code number
ICD10 Code: It displays the ICD10 code number
Ok : on click the CPT/HCPC popup is closed and the code(s) that is selected are added.

Code : The column displays the CPT or HCPC or the SNOMED code.
Seq. No.:
The column displays the sequence of tests.
SNOMED Code : The column displays displays the SNOMED code number. Mod : Enter an appropriate value for modifier. PrognoCIS can accept CPT / HCPC Codes with Modifiers. PrognoCIS Assessment has a provision of assigning one or more Modifiers to each CPT Code. Please Note, these are not assigned to HCPC codes. These codes influence the variance in charges to be considered. Group Master Type CM has a provision to define the modifier codes, along with applicable percentage. Units : Enter the dosage units.
BNG : Select an appropriate option Both/Non-Govt/ Govt from the drop-down list. Rev. Code : Enter the revenue code. Revenue codes are three digit numbers used on hospital bills, indicating to the Insurance Company, either the patient's place of treatment or the type of items received by Patient as part of his or her treatment. Ok: Click the button to assign a CPT or HCPC Code for the selected In House drug. Cancel : Click the button to cancel the process.
